If you would like more information about STOLEN WAGES please phone the Aboriginal Legal Service of WA (ALSWA) Info Line on FREECALL 1800 019 900.

If the WA Government kept any of your wages before 1972, ALSWA can provide you with more information about what assistance may be available to you.

Please contact ALSWA in relation to STOLEN WAGES by 6 September 2012.

If you would like to apply for Stolen Wages, please complete the attached forms.

Complete all the forms or if you need assistance contact your closest ALSWA Office.

Application Form
If people want to make an application, please use the attached official application form which is 6 pages in total (2 cover pages and 4 pages to fill in). The applicant will need to sign and a solicitor or court officer will need to witness each non-cover page (ie. pages 3, 4, 5 and 6) of the application form in the spaces provided.

Referee Form (Schedule 4)
If you have two forms of Identification you do NOT need to complete Schedule 4 (the referee forms) or get referees. It is only if you do not have ANY identification that you will need to get people who have known you more than a year to complete Referee Statutory Declaration.

Witness Statutory Declaration
This is a list of people that can witness a Statutory Declaration if you have completed the Referee Form.

Application for Birth Certificate
If you only hav one form of idendificatuion please complete the Application for Birth Certificate. Fill this form out and send it back to with your completed application and we will apply for and include your birth certificate with you application. This does NOT need to be completed if you have 2 forms of identification.
